Forms & Information



The first step to getting your child enrolled in our program is to fill out the enrollment form and pay the registration fee. Please visit the Enrollment page for more information. Once you get an acceptance email, please complete and send in all forms in order for your child to be considered enrolled in Dayton First Academy of Hope. All forms and questions can be sent to lesleigh.bagley@daytonfirst.com. The handbook will be uploaded as it is updated with information.
